  3. That is classic or traditional Medicare which is the 80/20 split which used to be the norm for insurance in general. Medicare prices are set by the government and if a doctor accepts assignment that means he is taking what Medicare offers and with a traditional policy the patient has a 20% co-pay. Because prices have been driven up so high by insurance corruption and government meddling, even 20% of lowered, controlled prices are a LOT, so most folks on Medicare either have a medigap policy which is yet another premium so they can get part of the 20% co-pay taken care of or they have an HMO or such to reduce their out-of-pocket expenses. This means they lose the choices they have under traditional Medicare.
